Arrival at the Clinic for your Appointment



If you arrive to the clinic as a New Patient, please arrive 20 minutes prior to your appointment time and:
  1. You will be required to provide proof of identity and your insurance card so we can file an insurance claim with your carrier on your behalf.
  2. You will be required to fill out a New Patient demographic and medical history questionnaire. (These forms can be downloaded from our website here and filled out prior to arriving in the clinic, which will help us process you to be seen more timely.)
  3. You will be required to pay any copay, coinsurance or deductibles that you are responsible for as a result of your insurance policy. Patients will be asked to reschedule if they are not prepared to make payments they are responsible for at the time of service.
If you arrive at the clinic as a Return Patient:
  1. You will be provided a printed demographic and medical history summery from our records that you will be required to review and update if necessary.
  2. You will be required to pay any copay, coinsurance or deductibles that you are responsible for as a result of your insurance policy. Patients will be asked to reschedule if they are not prepared to make payments they are responsible for at the time of service.
